Design features two tall rectangular shapes with multiple smaller window shapes above. Rectangular windows each feature six-point star at top; winged circle in oval window is featured above larger windows in center. Design is in shades green, brown, yellow, and blue.
Blue label with "Booth Art Glass Co." printed in white affixed to verso.
